This presentation is for students to demonstrate their understanding of course concepts regarding the summative understanding of disease/ health challenges.

Instructions 

1. Total time for the presentation is 20-25 minutes,
2. Presentation will consist of 
3. Part 1  (15%)
4. Identify the modifiable and non modifiable risk factors for the disease
5. Based on the modifiable and non modifiable risk factors identify initiatives to help prevent/manage the disease 
6. Part 2 (15%) 
7. Identify the at risk populations
8. How could decreased access to health care affect individuals with the disease?
9.  Part 3 (30%)
10. Based on peer reviewed articles create a plan of care outlining
11. 2 goals
12. for each goal 2 interventions
13. provide rationale/evidence for your interventions

15. What are some common themes that you noted with all of the disease processes?
